Use the Import function in
Linking plug-in (PDF) to import a CSV file that was previously exported for the purpose of updating the bookmarks and links in a PDF document.
Note: You cannot import a CSV file for bookmarks, links, or keywords during in-process linking.

Using the
Link and Bookmark Manager, under
Import, specify the
Output location where the exported CSV is stored.

Click
Import.
The imported bookmarks and hyperlinks appear in the
Link and Bookmark Manager dialog box. These bookmarks and links are now present in the PDF, but are not preserved until you save the document.

To save your changes, click
in Acrobat.
